Badger Prairie provides comprehensive person-centered and goal-oriented services. Our objective is to meet the needs of the whole person, including physical, social, intellectual, and spiritual needs.
In addition to our skilled nurses, we also staff MDs and psychiatrists who make rounds weekly.

Our newly constructed building opened on February 23, 2011. It features twelve (12) households, each with its own dining room, sunroom, and living room. Households also have private bedrooms, private bathrooms, and access to outdoor patios.
Badger Prairie offers two households with delayed egress and two locked households for those residents who need a higher level of supervision.
The nursing support areas throughout the care center are in discreet spots. This ensures they are not the dominant feature of resident life while still allowing the staff to work efficiently. We want to create a home-like environment and keep our residents feeling safe and comfortable.
Recreation Therapy uses recreation services and leisure experiences to help people with limitations enhance - physically, mentally, emotionally, and socially. Recreation Activity Therapy staff at BPHCC selects activity programs that are utilized to treat or maintain the physical, mental and emotional well being of individuals served. These interventions help individuals reduce the effects of illness or disability and achieve an optimal level of personal independence. The goals of interventions include improving physical, cognitive and social functioning.

Badger Prairie Health Care Center offers 24-hour skilled nursing care for residents who require long-term care or subacute services. We offer an excellent location and atmosphere and our dedicated staff welcome residents with a friendly smile and years of experience.

Medical care is provided by UW physicians. A comprehensive treatment plan is developed for each resident by our skilled staff of nurses, doctors and therapists. Our team meets complex medical needs, including wound care, pain management, rehab therapy, enteral feeding, hospice care and behavior management. A consulting psychiatrist sees each resident individually.
